**Remember, the Team Member will do the following:
Work with you (via text) to prepare for the 7 day booking window to open your your Lightning Lanes
Use the card in your account to purchase your Lightning Lane Multi Pass 7 days before you check in (for those staying on site)
Use the card in your account to purchase your Lightning Lane Single Pass for specific rides if you wish
Attempt to obtain a Virtual Queue Boarding group on the MORNING OF if you wish (only for select rides)
*If you are completing this form then you have ALREADY signed up & been confirmed for the Dad Assist Packagedays you would like AND your payment has been made to your team member.
*Please do NOT share this form with anyone outside of your immediate travel party as this is a package ONLY available to those who have booked through Family Adventures by the Dad, Disney with the Littles (DWTL), or another Once Upon a Times travel specialist.
*Once you have filled out your form, you may sit back and relax. Your team member will be in contact with you via text to the cell phone number you include.

You may skip any section you do not need (Filling this out twice, not using LL multi pass at a specific park, not wanting a specific LL Single Pass, etc)
If you are starting at a park more than once and wish to utilize your The Dad Assist Package, you will complete the last question in each section. If you are only visiting a park once, skip it. Fill it in as many times as necessary.
